Granville Vance Public Health is currently accepting applications for the position of Community Development Specialist I/Community Systems Change Consultant.

This is a consultative position to improve the community-wide systems of care that serve children and youth in Granville and Vance Counties. The primary purpose of this position is to support the sustainable achievement of Innovative Approaches’ action plan systems change objectives. The secondary purpose, when appropriate and needed, will be to support additional related projects within the Health Promotion & Wellness department. The purpose of the Innovative Approaches grant project is:

(1) to foster and support community strategies to create effective systems of care for families of children with special health care needs;

(2) to identify promising practices, and then;

(3) to help disseminate these practices to other communities.

This initiative aims to improve the community-wide systems of care to best meet the needs of families of children with special health care needs (CYSHCN). The consultant staffs and maintains a steering committee of community agencies and parents to assess system issues affecting families of CYSHCN, facilitates the development of an action plan driven by parent and provider data, facilitates the completion of action items, and assists with all evaluation efforts.

REQUIRED MINIMUM TRAINING and EXPERIENCE: Minimum qualifications for this position are graduation from a four-year college or university and three years of experience, preferably in a field related to the specific program assignment; or an equivalent combination of training and experience. Experience with community system change and community development or experience in management of community coalitions is strongly preferred.
