General Workplace Hazards

Walking and working surfaces free of trip hazards, spills, and obstructions

Adequate lighting in all work areas, stairwells, and exits

Electrical cords and equipment in good condition, no frayed wires or overloaded outlets

Proper ventilation and temperature control in occupied spaces

Hazardous materials properly labeled, stored, and accompanied by Safety Data Sheets (SDS)

Machine guarding in place on all equipment with moving parts

Noise levels assessed and hearing protection provided where required

Personal Protective Equipment (PPE)

PPE hazard assessment completed and documented for each job role

Appropriate PPE provided at no cost to employees (gloves, eyewear, hard hats, respirators)

Employees trained on proper use, maintenance, and limitations of assigned PPE

PPE regularly inspected and replaced when damaged or worn

Written PPE program on file and accessible to employees

Emergency Action Plan & Evacuation

Written Emergency Action Plan (EAP) posted and accessible to all employees

Evacuation routes clearly marked with illuminated exit signs

Fire extinguishers inspected monthly and serviced annually

Emergency contact numbers posted in visible locations

Evacuation drills conducted at least annually (more often for high-risk facilities)

OSHA Recordkeeping (300 Log)

OSHA 300 Log maintained for all recordable injuries and illnesses

OSHA 301 Incident Report completed within 7 calendar days of each recordable event

Annual summary (OSHA 300A) posted February 1 – April 30 each year

Records retained for 5 years following the end of the calendar year

Electronic submission to OSHA completed by March 2 (if 250+ employees or certain high-risk industries)

Employees informed of their right to access injury and illness records

Safety Training Documentation

New hire safety orientation completed before work begins

Hazard Communication (HazCom) training for employees who handle chemicals
